Abstract

¹æ¹ý: °Ç°­º¸Çè½É»çÆò°¡¿øÀÇ 2021³â ¿ä¾çº´¿ø ÀÔ¿ø±Þ¿© ÀûÁ¤¼º Æò°¡ °á°ú¿¡ ´ëÇØ 2Â÷ ÀÚ·á ºÐ¼®À» ¼öÇàÇÏ¿´´Ù. ÀÚ·á¼öÁýÀº 2023³â 6¿ù 19ÀϺÎÅÍ 6¿ù 27ÀϱîÁö ¼öÇàµÇ¾ú´Ù. ¿¬±¸´ë»ó ÀÇ·á±â°üÀº 1,215°³ º´¿øÀ̸ç, ¿¬±¸ÀÚ·á´Â SAS 9.4¿Í Mplus 8 ¼ÒÇÁÆ®¿þ¾î¸¦ È°¿ëÇÏ¿© ºÐ¼®ÇÏ¿´´Ù.

°á·Ð: ¿ä¾çº´¿øÀÇ °£È£»ç ¹èÄ¡¼öÁØ°ú ȯÀÚ°á°ú¸¦ °³¼±Çϱâ À§ÇÏ¿© °£È£»ç¿Í °£È£Á¶¹«»ç ¹èÄ¡¼öÁØÀ» º°µµ·Î »êÁ¤ÇÏ´Â Á¤Ã¥ ¼ö¸³À» Á¦¾ðÇÑ´Ù.

Purpose: The study aimed to examine the effects of nurse staffing levels on patient outcomes in long-term carehospitals in South Korea using path analysis.

Methods: We conducted a secondary analysis of national data, whichincluded the 2021 hospital assessment results of long-term care hospitals from the Health Insurance Review andAssessment Service. Data collection was performed between June 19 and June 27, 2023. The study sampleconsisted of 1,215 hospitals, and we analyzed the data using SAS 9.4 and Mplus 8 software.

Results: The averagenumbers of patients per registered nurse, certified nursing assistant, and nursing staff, including both registerednurses and certified nursing assistant, was 10.00, 7.43, and 4.00, respectively. Path analysis revealed that thenumber of patients per registered nurse had direct effects on improvements in activities of daily living and indirecteffects on indwelling catheterization, pressure ulcer improvement, and weight loss. The number of patients percertified nursing assistant had direct effects on new pressure ulcer development and pressure ulcer improvement,with no indirect effects. The number of patients per nursing staff had direct effects on pressure ulcer improvementand no indirect effects on other patient outcomes.

Conclusion: The findings suggest that establishing policies toevaluate staffing levels of registered nurse and certified nursing assistants separately is necessary in order toimprove registered nurse staffing levels and patient outcomes in long-term care hospitals.
